Objectives: To examine the experiences of spousal care givers of Alzheimer patients to disclosure of dementia diagnosis and subsequent care.Methods: A random sample of 1943 spousal care givers of people receiving medication for Alzheimer disease was sent a cross-sectional postal survey about their opinions on the disclosure of dementia and follow-up care. A smaller qualitative study included open-ended questions concerning their experiences of the same topics.Results: The response rate for the survey was 77%. Of the respondents, 1214 of 1434 acknowledged (...) themselves as their spouse’s care giver. The mean age of the care givers was 78.2 years, and that of demented spouses, 80.5 years. Of the care givers, 63% were women. The couples had long-lasting marriages. Of the care givers, 93% reported that dementia had been disclosed openly to their spouse; 97% also preferred that physicians openly inform the patients of the dementia diagnosis, although 55% of their spouses with AD had developed depressive symptoms after the disclosure. Of the care givers, 71% felt they had received sufficient information about dementia. However, only 50% estimated that their spouses’ follow-up care had been well organised. The responses in the qualitative study indicated that many care givers felt grief and anxiety. They also expressed feelings of loneliness and uncertainty about how to deal with follow-up care for dementia.Conclusions: Elderly spousal care givers were quite satisfied with the information given them about dementia. However, the support with regard to the follow-up care of care-giving families failed to meet their needs adequately. (shrink)

Progressive supranuclear palsy is a movement disorder with prominent tau neuropathology. Brain diseases with abnormal tau deposits are called tauopathies, the most common of which is Alzheimer's disease. Environmental causes of tauopathies include repetitive head trauma associated with some sports. To identify common genetic variation contributing to risk for tauopathies, we carried out a genome-wide association study of 1,114 individuals with PSP and 3,247 controls followed by a second stage in which we genotyped 1,051 cases and 3,560 controls for the (...) stage 1 SNPs that yielded P ≤ 10-3. We found significant previously unidentified signals associated with PSP risk at STX6, EIF2AK3 and MOBP. We confirmed two independent variants in MAPT affecting risk for PSP, one of which influences MAPT brain expression. The genes implicated encode proteins for vesicle-membrane fusion at the Golgi-endosomal interface, for the endoplasmic reticulum unfolded protein response and for a myelin structural component. © 2011 Nature America, Inc. The overall objective of this research is to determine the religious perceptions and its levels of university youth and to interpret the findings from the point of view of their psychological features. This research consists of four chapter. These are: introduction, first chapter consist of discussions of the basic concepts such as religiosity, spirituality, second chapter that the data obtained from the survey was assessed the relationships with hypotheses and conclusions in which the findings were interpreted. The study’s population consists (...) of the students at the undergraduate level in faculties of the Erciyes University in 2011-2012. A questionnaire was administered on 553 students, chosen randomly in this population. The survey contains descriptive and Likert style items. As independent variables in this study, branches of science, faculty, department, class, age, gender, living environment for the longest time before coming to university, graduated high school, mother's and father's education level and the accommodation place during training have been taken. When analyzing the data, percentages, frequency distributions, descriptive statistics, analysis of variance (one-way ANOVA) and T-Tests and Chi-Square (x²) techniques were used. As a result of this research hypotheses associated with gender, age, branches of science, faculty and department were confirmed on the other hand the others were rejected. (shrink)

Social scientific reasoning (SSR) is essential to social science education and to a democratic society as a whole. Students are challenged to analyze and reason about social problems such as social inequality, crime, and poverty. However, students experience difficulties with SSR. This study addresses the research question: Which design principles can guide teachers in designing lessons that promote social scientific reasoning? In this design-based research, four social science teachers employed a conceptualization of SSR and its levels together with three initial (...) design principles to develop curriculum materials and activities. These design principles and curriculum materials were piloted in two secondary education classes (9th and 10th grades) and evaluated by four social science teachers, four social science teacher educators and 90 students. The study produced six design principles that can promote students’ SSR. In combination with the curriculum materials, those design principles can help develop teachers’ pedagogical content knowledge and guide the design of tasks and units that develop SSR. (shrink)

The Monographs produced by the International Agency for Research on Cancer (IARC) apply rigorous procedures for the scientific review and evaluation of carcinogenic hazards by independent experts. The Preamble to the IARC Monographs, which outlines these procedures, was updated in 2019, following recommendations of a 2018 expert Advisory Group. This article presents the key features of the updated Preamble, a major milestone that will enable IARC to take advantage of recent scientific and procedural advances made during the 12 years since (...) the last Preamble amendments. The updated Preamble formalizes important developments already being pioneered in the Monographs Programme. These developments were taken forward in a clarified and strengthened process for identifying, reviewing, evaluating and integrating evidence to identify causes of human cancer. The advancements adopted include strengthening of systematic review methodologies; greater emphasis on mechanistic evidence, based on key characteristics of carcinogens; greater consideration of quality and informativeness in the critical evaluation of epidemiological studies, including their exposure assessment methods; improved harmonization of evaluation criteria for the different evidence streams; and a single-step process of integrating evidence on cancer in humans, cancer in experimental animals and mechanisms for reaching overall evaluations. In all, the updated Preamble underpins a stronger and more transparent method for the identification of carcinogenic hazards, the essential first step in cancer prevention. (shrink)

Conscious but incapacitated patients need protection from both undertreatment and overtreatment, for they are exceptionally vulnerable, and dependent on others to act in their interests. In the United States, the law prioritizes autonomy over best interests in decision making. Yet U.S. courts, using both substituted judgment and best interests decision making standards, frequently prohibit the withdrawal of life-sustaining treatment from conscious but incapacitated patients, such as those in the minimally conscious state, even when ostensibly seeking to determine what patients would (...) have wanted. In the United Kingdom, under the Mental Capacity Act of 2005, courts decide on the best interests of incapacitated patients by, in part, taking into account the past wishes and values of the patient. This paper examines and compares those ethicolegal approaches to decision making on behalf of conscious but incapacitated patients. We argue for a limited interpretation of best interests such that the standard is properly used only when the preferences of a conscious, but incapacitated patient are unknown and unknowable. When patient preferences and values are known or can be reasonably inferred, using a holistic, all-things-considered substituted judgment standard respects patient autonomy. (shrink)

One of the challenges facing AI governance is the need for multiple scales. Universal human rights require a global scale. If someone asks AI if education is harmful to women, the answer should be “no” regardless of their location. But economic democratization requires local control: if AI’s power over an economy is dictated by corporate giants or authoritarian states, it may degrade democracy’s social and environmental foundations. AI democratization, in other words, needs to operate across multiple scales. Nature allows the (...) multiscale flourishing of biological systems through fractal distributions. In this paper, we show that key elements of the fractal scaling found in nature can be applied to the AI democratization process. We begin by looking at fractal trees in nature and applying similar analytics to tree representations of online conversations. We first examine this application in the context of OpenAI’s “Democratic Inputs” projects for globally acceptable policies. We then look at the advantages of independent AI ownership at local micro-levels, reporting on initial outcomes for experiments with AI and related technologies in community-based systems. Finally, we offer a synthesis of the two, micro and macro, in a multifractal model. Just as nature allows multifractal systems to maximize biodiverse flourishing, we propose a combination of community-owned AI at the micro-level, and globally democratized AI policies at the macro-level, for a more egalitarian and sustainable future. Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. Additionally, chapters consider interreligious dialogue vis-à-vis theological anthropology in conciliar documents; openness to the spiritual practices of other faith traditions as a way of encouraging positive interreligious encounter; the role of lay and new ecclesial movements in interreligious dialogue; and the development of Monastic Interreligious Dialogue. We prove that in this case G is isomorphic to m.Bauval [1, Theorem V.2.1] proved that if a factorial ring A is elementarily equivalent to l[;X1,...,Xm], then A is isomorphic to F[X1,...,Xm], with F being the field of invertible elements of A.Our result is different from this property because, a priori, the field k is not the field of invertible elements . Furthermore, k[G]; is not always factorial or Neotherian: if G is isomorphic to the cartesian product ∏i<ω, then the number of irreducible elements that divide elements that divide X is infinite, and the ideal generated by the monomials X is not finitely generated.We recall that to be factorial of Neotherian are not elementary properties. Hypnotic responding might be due to attenuated frontal lobe functioning after the hypnotic induction. Little is known about whether personality traits linked with frontal functioning are associated with responsiveness to hypnotic suggestions. We assessed whether hypnotic suggestibility is related to the traits of self-control and impulsivity in 154 participants who completed the Brief Self-Control Scale, the Self-Regulation Scale, the Barratt Impulsiveness Scale , and the Harvard Group Scale of Hypnotic Susceptibility . BIS-11 non-planning impulsivity correlated positively with HGSHS:A . Furthermore, (...) in the best model emerging from a stepwise multiple regression, both non-planning impulsivity and self-control positively predicted hypnotic suggestibility, and there was an interaction of BIS-11 motor impulsivity with gender. For men only, motor impulsivity tended to predict hypnotic suggestibility. Hypnotic suggestibility is associated with personality traits linked with frontal functioning, and hypnotic responding in men and women might differ. (shrink)

Selon la façon dont on l’envisage et l’origine du regard qui se pose sur elle, l’autorité ne paraît pas représenter la même chose. Pour certains elle est l’expression d’un principe abstrait, atemporel, relatif au jeu des places qu’organisent les rapports sociaux, en l’occurrence les places spécifiques qu’occupent les parents par rapport à l’enfant. Pourtant, pour d’autres approches, notamment les approches psychanalytiques, l’autorité n’est pas un principe neutre, elle a un sexe, de préférence masculin. Que peut-on dire d’une telle affirmation à (...) notre époque de reconfiguration des relations non seulement entre les sexes mais aussi entre les générations, et plus globalement de l’ensemble des relations privées, si ce n’est des rapports entre le public et le privé? Je m’essaie ici à une analyse sociologique, alimentée de références psychologiques et historiques, de la signification de cette question et des réponses qu’il est possible de lui apporter. (shrink)

The question of providence and contingency in contemporary Western thought: a critical study from an Islamic perspective.

Mood profiling has a long history in the field of sport and exercise. Several novel mood profile clusters were identified and described in the literature recently ( Parsons-Smith et al., 2017 ). In the present study, we investigated whether the same clusters were evident in an Italian-language, sport and exercise context. The Italian Mood Scale (ITAMS; Quartiroli et al., 2017 ) was administered to 950 Italian-speaking sport participants (659 females, 284 males, 7 unspecified; age range = 16–63 year, M = (...) 25.03, SD = 7.62) and seeded k -means clustering methodology applied to the responses. Six distinct mood profiles were identified, termed the iceberg, inverse iceberg, inverse Everest, shark fin, surface, and submerged profiles, which closely resembled those reported among English-speaking participants ( Parsons-Smith et al., 2017 ). Significant differences were found in the distribution of specific mood profiles across gender and age groups. Findings supported the cross-cultural generalizability of the six mood profiles and offer new research avenues into their antecedents, correlates and behavioral consequences in Italian-language contexts. The history of the concept of “religion” in Western tradition has intrigued scholars for years. This important collection of eighteen essays brings further light to the ongoing debate. Three of the invited participants, W.C. Smith, M. Despland and E. Resolution is an effective deduction procedure for classical logic. There is no similar "resolution" system for non-classical logics (though there are various automated deduction systems). The paper presents resolution systems for intuistionistic predicate logic as well as for modal and temporal logics within the framework of labelled deductive systems. Whereas in classical predicate logic resolution is applied to literals, in our system resolution is applied to L(abelled) R(epresentation) S(tructures). Proofs are discovered by a refutation procedure defined on LRSs, that imposes (...) a hierarchy on clause sets of such structures together with an inheritance discipline. This is a form of Theory Resolution. For intuitionistic logic these structures are called I(ntuitionistic) R(epresentation) S(tructures). Their hierarchical structure allows the restriction of unification of individual variables and/or constants without using Skolem functions. This structures must therefore be preserved when we consider other (non-modal) logics. Variations between different logics are captured by fine tuning of the inheritance properties of the hierarchy. For modal and temporal logics IRS's are extended to structures that represent worlds and/or times. This enables us to consider all kinds of combined logics. (shrink)

BackgroundSocial networking sites (SNSs) have penetrated all aspects of health care professionals’ (HCPs’) professional and private lives. A new term, e-professionalism, has emerged, which describes the linking of traditional values with this new dynamic online environment for HCPs. The four aims of this study were: (1) to examine their SNS prevalence and usage habits, (2) to examine their perception of e-professionalism, (3) to develop an e-professionalism assessment compatibility index and (4) to investigate their tendencies and differences in values of the (...) e-professionalism assessment compatibility index (ePACI).MethodsA cross-sectional study was conducted among MDs and DMDs in Croatia via email using a questionnaire. The questionnaire was distributed to all MDs and DMDs who were members of their respective chambers. In addition to descriptive statistics, Chi-square or Fisher’s exact test when appropriate, t-test, ANOVA and Mann–Whitney U tests were used to determine differences between groups. A principal component analysis (PCA) with varimax rotation was used to investigate dimensionality. Results of the PCA were compared to the coding based on the Social Media e-Professionalism rubric in order to create the ePACI.ResultsOf the 1013 gathered responses, 753 were from valid SNS users and suitable for further analysis. Facebook (91.6%) and Instagram (63.1%) were the predominant SNSs used. Both groups have a good understanding of e-professionalism. The newly developed ePACI deviates slightly in the “conservative” direction in the cases of both, MDs (t506 = 19.033, p < 0.001) and DMDs (t245 = 12.991, p < 0.001). HCPs who are older (r = 0.308, p < 0.001), who have fewer SNS profiles (rs = −0.142, p < 0.001), and who access their profiles less frequently (r = −0.166, p < 0.001) have statistically significantly more conservative ePACI values.ConclusionsMDs and DMDs in Croatia are frequent SNS users, with Facebook and Instagram being the main SNSs used. Both groups have a good understanding of e-professionalism. The newly developed ePACI deviates slightly towards the conservative side regarding perception of the e-professionalism content for both groups. The development of the ePACI, and its subsequent usage in further research, will have a direct influence in its validation, gathering comparable data, and be able to direct efforts in oversight, regulation and education.
